I was at Steve & Barry's this weekend (they're having their annual "Everything's 8.99" sale) and found the PERFECT HFI/DFI shirt.
It's grey with a big pink cupcake on it. Inside the cupcake is a bunch of pink skulls. The hidden message being: "Sugar is Poison." My brother thought it was awesome, but it was only available in the girl section. Of course, I bought it for myself to spread our message, of course. ^_^

We've never heard of Steve and Barry's here in Northern CA but they were just featured on Project Runway-with Sarah Jessica Parker selling her clothes there. Sarah has a shirt that says "sugar" on the bottom and has a cartoon girl's face making a "Yuck" face. but, check this out: www.funtocollect.com/runtsbananajuniorshirt.html

I design t-shirts and supplies at Cafepress, but haven't designed any unsweetened shirts, yet. I'll have to think about it.

Sarah, I design MOSTLY obscure My Little Pony t-shirts. They keep flagging my designs and think I stole them from hasbro art, which is obnoxious. Fred, I like the Fructose-free shirt. I also liked the one that said "Food Intolerant-Don't Feed Me." I need to wear that one when I visit my mother-in-law. She insists on feeding me every time I go down, and all of her food is the pre-packaged microwave variety. Last time I made her make me plain rice with butter, and still she insisted on adding other stuff into it. Also, went back to Steve & Barry's. The shirt was still there. I think I'll take a picture of mine for reference.

I like the "Say no to Sugar" one. I also like Jane's idea - coming up with our own shirts, or at least creating ones to send our message. Laura, it sounds like you have the talent!! Come on now...

Ideas are always appreciated.
Really generic ideas I've thought up in the past couple minutes include:
Fruit is POISON (also Sugar is POISON or some imagery involving sugar/fruit and poison)
HFI Awareness means: Stop trying to feed me.
Pixie Stix are my hero
Given the choice of eating something just to appease you and consequently going through a couple days of agonizing pain and nausea, I think I'd rather just tick you off. (I'm not 100% happy with those last few words, but I like the rambling nature of this one.)
That's about it. To add onto that last submission, there's a bit of background. I actually got into a fight (and frequently do) with my mother-in-law who INSISTS on feeding me. And I INSIST on not eating anything she fixes. We got into quite the fight last time. Most, if not all, of her food has sugar in it in some format. She's not a label-reader. She also tries to feed my dog wheat-based food when she knows my dog is allergic to wheat. Frequently she's said, "Can't she have a waffle? Just one?"
I'm going to go bald because of this woman.

Similar spin to what's been said: "Don't Feed Me, I have HFI"
Laura, your MIL sounds EXACTLY like my FIL. He used to get soooo annoyed with me and my eating habits. He loves to cook and would be greatly offended when I couldn't eat something he made (which was 99% of the time). At least now I can clearly explain to him what is wrong and WHY I can't eat his food. He is starting to get it...sort of, but I can tell he is still greatly annoyed by my food "issues".

colormist, I sometimes use the peanut allergy as an example of how people react to different foods differently, maybe you could print out this article and explain to your mother-in-law that even though HFI is not an allergy it is still the body reacting to a particular substance. I can visualize a t-shirt that has a "flying" pixy stix with stick arms and legs and a cape. Maybe it has that on one side and the other side says Sugar kills or Sugar is evil or No sugar for me.
Or how about this... HFI means...this is the closest I get to fruit - and then show a stick guy/girl standing next to a big lemon.
Or maybe you could make an HFI "food pyramid"
